Your journey with us unfolds in three phases:  It begins with Discovery, where we identify any and all factors involved. Once we have a reasonable understanding of the “what?, how?, and why?” of pain, our team works closely with you to develop a personalized  Treatment plan.  After we achieve optimal treatment, we enter the Maintenance phase, where the patient understands what happened and how to manage their body going forward.  

Our patients have come to expect more extended initial visits than they would typically experience with other practices, as consultations are handled with more consideration and care. We also host a Pain Support Group for our patients on the last Wednesday of every month.

For your first visit, please bring:

  1. Government-issued ID
  2. All current medications in their original pill bottles
  3. Any other physician or radiology reports/CDs that you have
  4. Contact information for your Primary Care Provider, and other members of your care team
  5. Completed New Patient forms

If you anticipate opioid or Suboxone® therapy, please review our informed consent and policies. Please note, however, that opioids are rarely prescribed at the first visit.

Appointment guidelines

To keep appointments on schedule and running smoothly, we ask that you respect the following:

  • Please arrive 10 minutes early to complete paperwork
  • Expect to spend up to 2 hours with us.
  • All appointments must be scheduled 2+ hours in advance
  • No walk-ins accepted
  • Current patients may request an urgent visit by email or telephone.
  • $150 “No-Show” Fee Applied For:
    • Not showing up
    • Arriving more than 20 minutes late
    • Canceling with less than 24-hour notice
    • If you “no-show”, your appointment will be cancelled.  It is your responsibility to reschedule it.

General Policies

  • Medication Safety: For your safety, we need to keep your medication list up-to-date. Please bring all medication bottles or an updated list of prescribed medications every appointment.
  • Refills: Schedule next appointment in time for necessary refills – this is your responsibility
  • Behavior Policy: Rude or inappropriate behavior results in immediate discharge
  • Drug Screening: Urine drug screens may occur at any time to ensure your  safety
  • Communication: To deliver the best possible care, expect that we will communicate with your referring provider and primary care provider. Please bring your primary care doctor’s contact information.
  • Opioids: For those receiving opioids, please review our policies below:
  • Self-Pay: For self-pay patients, payment is expected prior to the start of the visit.
