For your first visit, please be prepared with the following:
Essential Documentation & Identification

Government-issued ID: A valid form of government identification is required for identity verification.

Active Insurance Card: You must bring your current insurance card.

Completed Intake Forms: All required new patient forms and financial agreements should be completed and returned, ideally 72 hours before your appointment to avoid delays or rescheduling.
Clinical Information & Records

Medication List: A detailed list of all current medications, including specific dosages and frequencies.

Physical Media: Bring a copy of your imaging on a CD (MRI, X-ray, CT scan, etc.).

Reports: Provide the written reports for any relevant imaging.

Digital Access: If you do not have a CD, you may provide a secure imaging link or online portal login.
Note: The office has direct access only to Lenox Hill Radiology and East River Imaging. -
Medical Records: Any other relevant clinical details, such as recent blood work or records from prior treatments.

Care Team Contacts: A list of any other providers you would like the practice to communicate with regarding your care.
Visit Preparation Tips

Arrival Time: Please arrive 15 minutes early to allow sufficient time for the check-in process.

Note-Taking: You are encouraged to bring a notepad or use your phone to record the visit for your personal reference, as these visits are comprehensive.
