A patient may arrive with years of records and several separate diagnoses. Cardiology is treating tachycardia. Gastroenterology is treating IBS-like symptoms or gastroparesis. Allergy is treating hives, flushing, or unexplained reactions. Psychiatry may have labeled the patient with anxiety. Rheumatology may have called the pain fibromyalgia.
Each label may describe one part of the experience. But sometimes, the larger pattern is missed: hypermobility or Ehlers-Danlos syndrome, mast cell activation symptoms, and POTS or dysautonomia occurring together.
Why the Triad Gets Missed
This connection is often missed because medicine is organized by specialty. Each clinician may focus on the system they know best: the heart rate, the gut, the skin reactions, the joint pain, or the anxiety-like symptoms.
But in many hypermobile patients, these symptoms do not exist in isolation. Connective tissue laxity can affect joint stability, blood vessel tone, posture, pain, and nervous system regulation. The result can look confusing when each symptom is treated as a separate problem.
What May Actually Be Going On
In hypermobility and Ehlers-Danlos syndrome, ligaments may not provide normal stability. Muscles often compensate by tightening, guarding, and working harder to protect the joints.
At the same time, some patients develop dysautonomia or POTS, where the body struggles to regulate heart rate, blood pressure, temperature, digestion, and blood flow with position changes. This can create racing heart, dizziness, fatigue, temperature sensitivity, brain fog, and a feeling of internal alarm.
Mast cell activation symptoms can add another layer. Patients may experience flushing, hives, swelling, food sensitivity, medication sensitivity, itching, GI symptoms, or inflammatory flares. Stress, pain, temperature changes, exertion, and nervous system activation may worsen symptoms.
Why Treating One Piece May Not Be Enough
If the joint instability, immune reactivity, and autonomic dysfunction are all interacting, treating only one symptom may not create lasting improvement.
A more effective workup starts by looking for the pattern. Which symptoms came first? What triggers flares? How do posture, meals, exertion, stress, temperature, and pain affect the body? What is mechanical, what is inflammatory, and what is nervous-system driven?
