For many patients with Ehlers-Danlos syndrome, the path to diagnosis is long and frustrating. Symptoms may begin in childhood or early adulthood, but the diagnosis may not come for years, sometimes decades.
This delay happens because EDS is not limited to one body part. It can affect joints, muscles, nerves, blood vessels, digestion, the autonomic nervous system, and immune reactivity. When each symptom is evaluated separately, the larger connective tissue pattern can be missed.
What Patients Are Often Told
Patients with EDS are often given labels that describe part of the problem but do not explain the whole picture.
Widespread pain and fatigue may be labeled as fibromyalgia or chronic fatigue. Dizziness, racing heart, and internal alarm may be called anxiety. Joint pain may be treated as isolated orthopedic injuries. Digestive symptoms may be managed as reflux, constipation, IBS, or gastroparesis.
These labels may reflect real symptoms, but they can miss the unifying driver: connective tissue laxity and the body’s attempt to compensate for unstable joints and stressed systems.
Why Normal Testing Can Be Misleading
Many patients are told that scans look normal. But standard imaging is usually performed while the body is lying still and relaxed. Hypermobility is often dynamic. A joint or cervical spine segment may shift under gravity, movement, or fatigue in ways that are not visible on a routine scan.
The physical exam matters. Joint range of motion, instability, skin and tissue findings, history of injuries, family history, autonomic symptoms, and multisystem patterns all help guide the diagnosis.
Why Stiffness Can Hide Hypermobility
One of the most confusing parts of EDS is that patients may appear stiff, not flexible.
When ligaments are too loose to provide stability, muscles often tighten to protect the joints. Over time, this guarding can create severe stiffness, spasms, fatigue, and pain. A patient may no longer look obviously hypermobile, even though the underlying problem is still ligament laxity and joint instability.
What a Proper Workup Requires
A proper EDS workup has to connect the pattern across systems.
It should ask: Are there recurrent sprains, subluxations, joint injuries, pelvic or spine instability, POTS symptoms, mast cell activation symptoms, GI dysmotility, migraines, or widespread pain that does not fit one isolated diagnosis?
The physical examination is thorough and qualitative. Patterns will be recognized (or not).
Imaging for dynamic instability, and blood work for frequently related inflammatory conditions are included. Genetic testing is especially helpful for identifying mutations that can present like hEDS but are actually something else (present in up to 26% of people with hEDS criteria), or more significant EDS variants.
