When pain becomes chronic, it can stop behaving like a simple warning signal. Over time, the nervous system may become more reactive, interpreting normal sensations as threatening and amplifying pain signals before they fully reach conscious awareness.
This process is called central sensitization.
It does not mean the pain is imaginary. It means the nervous system has become trained to stay on high alert.
A Nervous System Alarm That Becomes Too Sensitive
Pain is produced by the brain after it receives and interprets danger signals from the body. In a healthy system, those signals help protect us from injury.
But when the nervous system receives repeated distress signals over time, the alarm system can become too sensitive. Pathways in the spinal cord and brain become more practiced at transmitting pain. The body may begin reacting to smaller triggers with larger pain responses.
It is similar to a home alarm system set so high that a small breeze triggers the siren.
What Sensitization Can Feel Like
Sensitized pain often feels different from a typical injury. It may be widespread, moving, burning, prickling, heavy, or difficult to localize.
Patients may experience allodynia, where light touch, clothing, temperature changes, or pressure feel painful. They may also experience hyperalgesia, where a painful stimulus feels much stronger than expected.
Central sensitization can also involve the autonomic nervous system. Symptoms may include sweating changes, temperature sensitivity, skin color changes, bowel or bladder changes, poor sleep, brain fog, and a physical sense of internal alarm.
What Drives Sensitization
Central sensitization rarely appears without a reason. It is often fueled by ongoing mechanical, neurologic, inflammatory, or emotional stress on the body.
A persistent nerve injury, unstable joint, herniated disc, pelvic pain, endometriosis, chronic constipation, migraine, autoimmune inflammation, MCAS, or post-infectious syndrome can keep sending distress signals into the nervous system.
Medical trauma, fear, exhaustion, and years of being dismissed can also make the brain’s threat system more reactive.
What a Proper Workup Requires
The challenge is that sensitization can create so much “noise” that the original pain generator becomes harder to see.
A proper workup has to do two things at once: calm the overactive nervous system and continue searching for the underlying driver. Treatments may include pain psychology, biofeedback, medication management, sympathetic nerve blocks, IV lidocaine, ketamine, or targeted procedures when appropriate.
Once the nervous system is quieter, the care team can better identify the structural, inflammatory, or neurologic source that may be keeping the pain cycle active.
