A Deeper Look at Tarsal Tunnel Syndrome and Foot Nerve Pain

Tarsal tunnel syndrome is a tibial nerve entrapment condition, but burning or tingling foot pain can have several possible causes.

Tarsal Tunnel Syndrome

Tarsal tunnel syndrome occurs when the tibial nerve becomes compressed or irritated as it passes through the tarsal tunnel near the inside of the ankle. The tibial nerve and its branches help provide sensation and motor function to parts of the foot. When this nerve is irritated, patients may feel burning, tingling, numbness, electric pain, shooting pain, or altered sensation.

Symptoms may affect the inside of the ankle, heel, arch, sole, or toes. Some patients notice symptoms during walking or standing. Others feel burning or tingling at night, after exercise, or after prolonged activity.

Although tarsal tunnel syndrome is a recognized cause of foot nerve pain, similar symptoms can come from many other conditions. A diagnosis-first evaluation helps determine whether the tibial nerve is the main pain generator or whether symptoms are coming from another local, systemic, or spine-related source.

What Tarsal Tunnel Syndrome Can Feel Like

Tarsal tunnel syndrome can feel like burning, tingling, numbness, electric pain, sharp pain, or nerve irritation in the bottom of the foot or ankle. Symptoms may travel into the arch, heel, sole, or toes. Some patients describe a pins-and-needles sensation or a feeling that the foot is irritated from the inside.

Pain may worsen with standing, walking, running, certain shoes, swelling, ankle position, or pressure around the inside of the ankle. Some patients feel symptoms after activity, while others notice pain at rest or during sleep.

The exact symptom pattern matters because it helps distinguish tarsal tunnel syndrome from plantar fasciitis, peripheral neuropathy, Morton’s neuroma, metatarsalgia, lumbar radiculopathy, and other causes of foot pain.

Why Tarsal Tunnel Syndrome Happens

Tarsal tunnel syndrome may occur when the tibial nerve is compressed or irritated by surrounding structures. Possible contributors include swelling, flat feet, ankle instability, scar tissue, cysts, varicose veins, tendon irritation, arthritis, trauma, inflammatory disease, or repetitive stress.

Foot and ankle mechanics can also matter. If the ankle collapses inward, the arch is unstable, or the foot absorbs load unevenly, the tissues around the tarsal tunnel may become irritated. Recurrent sprains or chronic ankle instability can also contribute to local nerve sensitivity.

In patients with hypermobility spectrum disorder or Ehlers-Danlos syndrome, joint laxity and altered mechanics may increase stress through the foot and ankle. This can contribute to tendon strain, ligament irritation, recurrent sprains, and nerve irritation in selected patients.

Tarsal Tunnel Syndrome vs. Plantar Fasciitis

Tarsal tunnel syndrome and plantar fasciitis can both cause pain in the heel or bottom of the foot, but they involve different structures. Tarsal tunnel syndrome involves the tibial nerve. Plantar fasciitis involves the plantar fascia, a band of tissue that supports the arch of the foot.

Plantar fasciitis often causes heel pain with first steps in the morning or after sitting. Tarsal tunnel syndrome is more likely to cause burning, tingling, numbness, or electric pain. However, symptoms can overlap, and some patients may have both conditions or another contributor entirely.

MPM evaluates the location, symptom quality, nerve pattern, mechanical triggers, tenderness, and prior treatment response to help clarify the pain source.

Tarsal Tunnel Syndrome vs. Peripheral Neuropathy

Peripheral neuropathy can also cause burning, tingling, numbness, or electric pain in the feet. Unlike tarsal tunnel syndrome, which is usually a focal entrapment near the ankle, peripheral neuropathy often affects nerves more broadly. Symptoms may be more diffuse, symmetric, or present in both feet.

Some patients with peripheral neuropathy may also develop focal nerve irritation. Others may have symptoms that resemble tarsal tunnel syndrome but are actually coming from diabetes-related neuropathy, small fiber neuropathy, autoimmune neuropathy, vitamin deficiency, medication effects, or another systemic condition.

MPM evaluates whether the symptom pattern suggests local nerve compression, broader neuropathy, spine-related nerve pain, or a mixed picture.

Tarsal Tunnel Syndrome vs. Morton’s Neuroma and Metatarsalgia

Morton’s neuroma usually causes localized forefoot nerve pain, often between the toes, and may feel like burning, tingling, numbness, or the sensation of stepping on a pebble. Metatarsalgia is a broader term for pain and inflammation in the ball of the foot.

Tarsal tunnel syndrome usually affects the tibial nerve near the inside of the ankle and may cause symptoms along the bottom of the foot, arch, heel, or toes. The location and nerve distribution are different, but patients may describe all of these conditions as burning or tingling foot pain.

A careful exam and diagnostic testing when appropriate can help determine whether symptoms are coming from the ankle-level tibial nerve, the forefoot, a joint, a tendon, or another source.

Hypermobility, EDS, and Ankle or Foot Nerve Irritation

Hypermobility and EDS can complicate foot and ankle pain. Joint laxity may affect how the ankle stabilizes, how the arch absorbs load, and how pressure moves across the foot. This can contribute to recurrent sprains, chronic tendon irritation, joint pain, metatarsalgia, Morton’s neuroma-like symptoms, or tarsal tunnel-type nerve irritation.

Patients with hypermobility may also compensate by tightening muscles or changing gait patterns, which can place additional stress on the foot, ankle, knee, hip, pelvis, and lower back.

MPM considers these relationships carefully. The goal is not to assume that hypermobility causes every symptom, but to understand whether joint mechanics, nerve irritation, and pain sensitivity are interacting.

Why Tarsal Tunnel Syndrome Can Be Difficult to Diagnose

Tarsal tunnel syndrome can be difficult to diagnose because symptoms may be intermittent, positional, or similar to other foot and nerve conditions. Imaging may be normal, or testing may not fully explain the patient’s symptoms. Some patients have tried orthotics, stretching, rest, medication, or physical therapy without a clear answer.

Tarsal tunnel-like symptoms can overlap with plantar fasciitis, peripheral neuropathy, lumbar radiculopathy, tendon injury, ligament injury, arthritis, gout, vascular disease, inflammatory disease, diabetic complications, or complex regional pain syndrome.

A diagnosis-first evaluation helps determine whether the pain should be treated locally at the foot and ankle, evaluated as a broader neuropathy, assessed through the spine, or coordinated with another specialist.

How MPM Evaluates Tarsal Tunnel Syndrome

MPM begins with a detailed history and physical exam. The evaluation reviews symptom location, burning or tingling patterns, numbness, weakness, ankle and foot mechanics, swelling, prior sprains, footwear triggers, walking tolerance, prior imaging, orthotic response, and previous treatments.

The exam may assess nerve sensitivity, sensation, strength, tenderness along the tarsal tunnel, ankle stability, foot posture, tendon irritation, gait, and signs of inflammatory or neuropathic involvement. Prior X-rays, MRI, ultrasound, EMG, nerve conduction studies, or spine imaging may be reviewed when available.

Diagnostic ultrasound may be considered when the clinical question involves the tibial nerve region, nearby tendons, cysts, soft tissue changes, swelling, or other structures that may contribute to nerve irritation.

Treatment Options for Tarsal Tunnel Syndrome

Treatment depends on the suspected cause and severity of symptoms. Conservative options may include activity modification, footwear changes, orthotic coordination, ankle support, physical therapy coordination, medication management when appropriate, and strategies to reduce mechanical irritation.

When the evaluation suggests a specific nerve, tendon, joint, or inflammatory target, ultrasound-guided injections may be considered. Steroid injections may be considered in selected cases when inflammation is likely and the risk profile is appropriate. Peripheral nerve blocks may be considered for selected nerve pain patterns.

Regenerative options such as PRP or prolotherapy may be considered only when a specific musculoskeletal contributor is identified and after careful review of diagnosis, anatomy, goals, risk, and evidence. These are not routine treatments for every case of tarsal tunnel syndrome.

When Nerve Hydrodissection or Surgery May Be Considered

Nerve hydrodissection may be considered only in selected entrapment patterns when anatomy and clinical findings support that approach. It is not a universal treatment for burning foot pain and should not be presented as a guaranteed alternative to surgery.

Surgical referral may be appropriate when symptoms are severe, persistent, progressive, associated with weakness or nerve damage, or caused by a structural compression that may require decompression. MPM coordinates referral when podiatry, orthopedics, peripheral nerve surgery, neurology, or another specialist is needed.

When Symptoms Require Urgent Evaluation

Patients should seek urgent evaluation for sudden inability to bear weight, severe swelling, deformity, open wound, spreading redness, fever, a hot swollen joint, sudden severe pain, new numbness or weakness, color or temperature change in the foot, loss of pulses, diabetic foot wound, suspected infection, trauma, bowel or bladder dysfunction, or rapidly worsening symptoms.

Tarsal tunnel-like symptoms can overlap with peripheral neuropathy, lumbar radiculopathy, plantar fasciitis, Morton’s neuroma, metatarsalgia, tendon injury, ligament injury, arthritis, gout, vascular disease, infection, diabetic complications, and complex regional pain syndrome. Red flags should not be ignored.
