Vasculitis can affect blood vessels throughout the body, which is why symptoms, diagnosis, urgency, and treatment vary widely.

Vasculitis

Vasculitis is not one single disease. It is a group of rare inflammatory blood vessel disorders that can affect arteries, veins, or capillaries anywhere in the body. When blood vessels become inflamed, the vessel wall may thicken, narrow, weaken, or become damaged. If blood flow is restricted, tissues and organs may not receive the oxygen and nutrients they need. This is why vasculitis can range from mild skin or joint symptoms to organ-threatening disease.

For some patients, vasculitis begins with symptoms that feel vague: fatigue, fever, weight loss, weakness, muscle aches, joint pain, or a general feeling of being unwell. For others, the first clue may be a rash, numbness, headaches, sinus symptoms, shortness of breath, abdominal pain, or abnormal kidney testing. Because symptoms depend on which vessels and organs are involved, vasculitis diagnosis often requires careful pattern recognition and coordination across specialties.

What Vasculitis Symptoms May Look Like

Vasculitis symptoms can involve many systems. Skin involvement may cause red or purple spots, nodules, ulcers, or other rashes. Joint and muscle involvement may cause aching, swelling, stiffness, or diffuse pain. Nerve involvement may cause numbness, tingling, burning pain, weakness, or changes in sensation. Sinus or airway involvement may cause nasal symptoms, cough, shortness of breath, or chest discomfort. Some patients may have headaches, vision changes, abdominal pain, blood pressure changes, or kidney findings such as blood or protein in the urine.

These symptoms can overlap with lupus, rheumatoid arthritis, Sjogren’s syndrome, undifferentiated connective tissue disease, post-COVID pain, fibromyalgia, peripheral neuropathy, headache and migraine, spine pain, and other complex pain conditions. This overlap is one reason vasculitis should not be diagnosed based on symptoms alone.

What Causes Vasculitis?

The causes of vasculitis vary. Many forms involve immune system inflammation, where the immune system contributes to blood vessel injury. Some cases may be associated with infections, medications, cancers, or other autoimmune diseases. Some forms have no clearly identified trigger. ANCA-associated vasculitis is one category in which specific immune markers may help guide diagnosis and treatment, but ANCA testing is not relevant to every form of vasculitis.

The key clinical question is not only whether a patient has vasculitis, but what type, which vessels are involved, which organs may be affected, and how active or severe the disease appears. This determines whether care can be managed with routine specialist evaluation or requires urgent treatment.

How Vasculitis Is Diagnosed

Vasculitis diagnosis usually combines several types of information. A clinician may review symptoms, timing, flare pattern, medication exposure, infection history, autoimmune history, family history, physical examination, blood work, inflammatory markers, kidney function, urinalysis, imaging, and sometimes biopsy. Imaging may be used to assess blood vessels or organ involvement. Biopsy may help confirm inflammation in certain tissues when clinically appropriate.

At MPM, evaluation also considers pain drivers. A patient with vasculitis may have pain from active inflammation, but also from nerve involvement, joint inflammation, tissue injury, tendon irritation, musculoskeletal compensation, or chronic pain sensitization. Diagnostic ultrasound may help assess certain joints, tendons, soft tissues, or injection targets when appropriate, but it is only one part of a broader evaluation.

Treatment Options for Vasculitis

Treatment depends on the vasculitis type, severity, and organ involvement. Corticosteroids are commonly used to control active inflammation, especially when disease is significant. Other medications may include steroid-sparing immunosuppressants, DMARDs, biologics, or other immune-directed therapies. The goal is often to control active inflammation, protect organs, reach remission when possible, and reduce relapse risk.

These medications can carry meaningful side effects, including infection risk, metabolic changes, bone effects, and other medication-specific concerns. They require physician supervision and monitoring. Patients should not stop, start, or change steroids, immunosuppressants, DMARDs, or biologics without medical guidance.

At MPM, treatment planning focuses on vasculitis-related pain and overlapping pain conditions. Depending on the clinical picture, care may include medication management, infusion coordination, biologics when appropriate, peripheral joint injections, sacroiliac joint injection, steroid injections for selected pain generators, diagnostic ultrasound, B12 replacement when deficiency is relevant, weight-management support, pain psychology, and coordination with physical therapy. These treatments are not presented as cures for vasculitis. They may support pain control and function as part of a coordinated medical plan.

When Vasculitis May Be Urgent

Some vasculitis symptoms require prompt medical attention. Breathing difficulty, chest pain, severe headache, vision changes, neurologic symptoms, severe abdominal pain, dark urine, blood in urine, rapidly worsening rash, marked weakness, fainting, or sudden numbness or weakness should be evaluated urgently. These symptoms can have multiple causes, but they may indicate organ involvement or another serious condition.

Early evaluation is especially important because some forms of vasculitis can affect the kidneys, lungs, brain, nerves, heart, or major blood vessels. Pain management should never replace urgent evaluation when organ-threatening symptoms are present.

Why Pain Can Persist With Vasculitis

Some patients continue to experience pain after active inflammation is treated. This does not mean the pain is not real. It may mean that nerve irritation, tissue injury, musculoskeletal compensation, headaches, spine pain, sacroiliac joint dysfunction, fibromyalgia, medical PTSD, deconditioning, or centralized pain sensitivity has become part of the symptom picture.

MPM evaluates whether pain appears inflammatory, neuropathic, musculoskeletal, centralized, or multifactorial. This helps avoid treating every symptom as active vasculitis while also avoiding the opposite problem, which is dismissing pain when routine testing does not explain the full burden.
